HOARDING INTERVENTION

Chapter: Operational Guidelines Revised: 05/25/06
I. It shall be the responsibility of all Fire Department personnel to be familiar with the ideas, principals, and procedures contained in these guidelines.
II. Intervention needs to be collaborative involving the individual, family and other agencies, mental health, adult protective services, fire prevention, building and safety, animal control and TPD, if necessary.
III. Mandatory Reporting Requirement.
A. Hoarding or self-neglect is a mandatory reporting requirement to Los Angeles County Adult Protective Services (APS).
B. For individuals 60 years of age or older, a referral form should be sent to GENESIS.
C. Contact the Department of Children and Family Services (DCFS) if children are involved.

V. Definitions:
A. APS-Adult Protective Services. Investigate all situations involving elders (65 or older) or dependant adults (physically or mentally impaired).
B. DCFS-Department of Children and Family Services.
C. Hoarding is the excessive collection and retention of things or animals until they interfere with day-to-day functions such as home, health, family, work, and social life. Severe hoarding causes safety and health hazards.
D. Mandated Reporter – Fire Department personnel, Law enforcement, home health agencies, clinics, foster homes, residential care facilities for the elderly. A more comprehensive list identifying mandated reporters can be found at: www.dss.cahwnet.gov/pdf/soc341.pdf.
E. Self-neglect is the failure to exercise a reasonable degree of care in providing his/her own needs in such areas as personal hygiene, food, clothing, shelter, medical and mental health care, or avoiding health and safety hazards, malnutrition or dehydration, when that failure is due to poor cognitive functioning, mental limitations, substance abuse or chronic poor health.
VI. Safety Issues
A. Masks, gloves and booties should be worn in homes with severe hoarding behaviors or when animal hoarding is discovered.
1. Listeria, Hepatitis A and B, Scabies, Pneumonia, and Shigella are just a few of the diseases transmitted via human to human.
2. Tapeworm, hanta virus, psittacosis, and cat scratch disease are but a few of the diseases that can be passed animal to human.

VII. Procedures
A. Regular Business Hours.
1. Contact the Fire Prevention Area Inspector while on-site. This allows the area inspector to immediately enter the property for inspection and follow through.
B. After Hours.
1. Notify the Area Fire Inspector of the hoarding situation via email. Provide copies of the Incident Report, APS referral form and GENESIS form, if applicable.
C. Take pictures of the condition of the home, if time allows. Taking pictures of the resident may conflict with HPPA laws. (Should verify this info with Torrance legal).
D. Complete APS form.

E. Complete GENESIS referral form, if applicable. (Need to attach document to SOG)

Overview

Approximately 12-15 Task Force Members were recruited representing various backgrounds and organizations, including landlords (non-profit landlords, San Francisco Apartment Association), city departments (including DAAS, CBHS, City Attorney’s Office, Mayor’s Office on Disability), legal, mental health, and others. Task Force members give presentations to the group on their area of expertise (e.g. indicators for unsafe conditions, guidelines for working with people who hoard, and who to contact for help). The Task Force coordinates its efforts with the work of two groups that are currently meeting: a group of City employees that meets to discuss specific hoarding cases and the Mental Health Association of San Francisco’s (MHA-SF) annual hoarding conference planning committee.
The Task Force membership and goals were developed and refined at the first two meetings. The Task Force will meet six times, every other month during the first year. In addition, a planning committee of 4-6 members was developed to plan the Task Force Meetings. After the first year, members of the Task Force will determine whether there is need for the Task Force to continue to meet, and if so, how often they should do so to realize any new goals they set forth.
Task Force Goals:
• Identify gaps and barriers in service
• Assess current services and needs
• Identify best practices to improve coordination of services and eviction prevention to reduce eviction and
improve quality of life for compulsive hoarders
• Raise awareness among the public and policy makers

The Institute on Compulsive Hoarding and Cluttering (ICHC) is pleased to announce that a new treatment group is starting April 28th, from 6pm to 8pm, and will be held every Monday evening thereafter for 16 weeks.
This treatment group is unique because it is the first treatment group of its type that is available in the Bay Area, designed to specifically treat the behaviors associated with compulsive hoarding and cluttering. The Mental Health Association of San Francisco (MHA-SF) and Family Service Agency of San Francisco are collaborating to provide this opportunity to those individuals who meet criteria for the group.
